Advanced Eye Care Center, Inc. is a comprehensive, state of the art, ophthalmology practice providing medical and surgical eye care to patients in a convenient Mansfield, Ohio location.

Harold A. Ballitch II, M.D., FACS is a board certified ophthalmologist who evaluates and treats conditions including, but not limited to, cataract, glaucoma, diabetes, dry eyes, ptosis (droopy eyelids), and routine eye exams. Together with his well trained clinical staff of technicians and nurses, he strives to provide patients with the most technologically advanced and highest quality medical and surgical care available. His attention to detail and holistic approach to caring for patients serves to provide individualized evaluation and treatment for patients.

Harold A. Ballitch II, M.D., FACS

Ophthalmologist Mansfield | Dr. Harold A. Ballitch II, M.D. Harold A. Ballitch II, M.D., FACS is a Board Certified Ophthalmologist practicing in the Mansfield area since 1992. A graduate of Michigan State University Medical School, he completed residency at the Medical College of Virginia where he served as chief resident. He then accomplished an Oculoplastic Fellowship in Albany, New York, specializing in eyelid and orbital reconstruction.

Dr. Ballitch is a Fellow of the American Academy of Ophthalmology and a Fellow of the American College of Surgeons with a subspecialty in Ophthalmic (Eye) Plastic Surgery.

Gurpinderjeet Kaur, O.D.

Ophthalmologist Mansfield | Dr. Kaur received her undergraduate degree in Physiology from University of Washington in Seattle, and earned her doctorate degree from Pennsylvania College of Optometry in Philadelphia, Pennsylvania in 2011. She also completed residency at the Louis Stokes VA Medical Center in Cleveland where she received extensive training in management of ocular pathology including diabetic retinopathy, glaucoma, and macular degeneration.

Dr. Kaur is an active member of the optometry community. She has served as an adjunct faculty at the Ohio College of Optometry. She is an active member of American Academy of Optometry, American Optometric Association and Ohio Optometric Association. Dr. Kaur provides full scope optometric care including geriatric and pediatric services, low vision evaluation and specialty contact lenses.

Advanced Eye Care Center, Inc. was established in 1997 by Harold A. Ballitch II, M.D., FACS. Our medical office and AAAHC certified ambulatory surgery center are located at 1991 Park Avenue West, Mansfield, Ohio. Patients are seen by appointment.

About Your Appointment

For the safety of you and others, a driver is recommended for any appointment that requires your eyes to be dilated. Generally speaking, this will be for any new patient exam, annual eye exam, and on a regular basis for certain eye conditions (i.e. Macular Degeneration, Diabetes). If you are uncertain whether or not your eyes will be dilated, you may call the office for clarification.

PLEASE BRING THE FOLLOWING ITEMS TO EACH VISIT:

Your health insurance card(s)
Photo ID (i.e. driver’s license)
Your medical insurance co-payment
Updated list of medications and information regarding any health changes, procedures, or hospitalizations

Insurance and Billing

Our office is committed to making your visit a pleasant one. As a courtesy to you, we are happy to assist you with filing your insurance claim(s). We will file primary and secondary only. Any tertiary claims are your responsibility. We are a medical office, and we also participate with vision insurance (i.e. VSP, EyeMed)

At each visit, your address, phone number, and insurance information will be verified.

Your insurance plan may require a referral. You are responsible for obtaining this referral from your primary care physician. If you are seen without the proper authorization, you may be responsible for the charges incurred. Please be familiar with your coverage.

Covered Services

Insurance plans vary widely, and many of them have limits or conditions to coverage. In some cases, certain procedures may not be covered. If your insurance does not cover these services, you may be personally and fully responsible for payment. Commonly these limits include:

REFRACTIONS (this determines the prescription for glasses, and is sometimes done to determine the best visual acuity for medical or diagnostic purposes )
ROUTINE OR PREVENTIVE EYE EXAMINATIONS

We will be happy to bill your insurance, but ask that you pay the refraction fee in full at the time of service since this is most commonly not a covered service. If your insurance does remit payment, you will receive a refund from our office.
