Our office is open from 7:45 am to 5:00 pm, Monday through Friday. Telephone calls are handled by trained members of our staff who can assist you with your questions. Dr. Ballitch is also available and would be glad to speak with you if your question or problem cannot be handled by staff members.

Your visit with us may include a complete eye evaluation. For this, both eyes may be dilated. Your vision may remain blurry for several hours. For your safety, we strongly suggest coming with someone to drive you home. If specialized tests are required, we try to complete them at the time of your visit. Please allow at least one hour for your initial consultation.
Please remember to:
- Bring your glasses, contact lenses and contact lens case.
- Take your usual medications, including eye drops prior to your appointment time, or bring them with you if you need to take them in our office.
- Bring a list of all medications you are taking, including how often you take them.
- Bring your health insurance cards, Medicare card and your social security number.
- Diabetic patients should especially remember to eat their regular meals.
Our fees reflect the level of care you will receive. We have standardized charges for each procedure. Our fees are listed in the business office and are available to you. If you have a question about fees, please discuss them with our insurance staff. We participate in most insurance plans and accept assignment on Medicare claims. While you are responsible for the charges for the services rendered, we will file the charges with your insurance carrier.
If your insurance requires a referral from your primary care physician, please contact them prior to your appointment. If our fees impose a financial burden on you, we will work with you on a payment plan. We do not want to create a hardship for you or your family. If you have no health insurance, please discuss this in advance with our insurance staff who may be able to help you obtain assistance.
If you are hospitalized, or undergo surgery, consulting physicians such as internists and anesthesiologists may participate in your care. You will receive a statement directly from the hospital and from the consulting offices for their professional services. If you have questions regarding their statements, please call them directly.
